Forfeiture of bonus will occur if negative safety events such as, but not limited to, preventable accidents and/or moving violations take place.• A total of $500 will be payable to a current PK field employee who refers a new worker to PK Contracting. • This form must be completed in its entirety and submitted to Human Resources within 90 days of hire. • The referred individual must be with PK for a full season and return for the following season before payment will be made. • The safety record of the person referred will be reviewed during the bonus program period.
